Complex, Multi-Stage Cardiac and Vascular Surgery successfully performed on 60-year-old male at Kokilaben Dhirubhai Ambani Hospital Rare Surgical feat offers hope for patients with Complex Cardiovascular Disease

Navi Mumbai, July 11, 2024 (GNI): Surgeon at Kokilaben Dhirubhai Ambani Hospital, Navi Mumbai have successfully performed a complex, multi-stage combined cardiac and vascular procedure on a 60-year-old male patient suffering from severe blockages in his coronary arteries as well as arteries in his lower limbs. The patient, who suffered from intermittent chest pain and severe pain in the legs, was diagnosed with critical calcified triple vessel disease with complete blockage of two important blood vessels. A combination of coronary artery bypass grafting (CABG) and extensive peripheral artery bypass surgery was performed to address multiple critical blockages throughout the patient’s vascular system.

“This was a particularly challenging case due to the extent and severity of the blockages throughout the patient’s vascular system,” said Dr. Amjad Shaikh, Consultant Cardiovascular Surgeon at Kokilaben Dhirubhai Ambani Hospital, Navi Mumbai. “The patient presented with severe pain in his left leg along with chest pain. He was a known case of diabetes and had a history of heavy smoking for over 30 years. Diagnostic tests including ECG, angiography and Doppler revealed critical blockages in his coronary arteries, the aorta, and the arteries supplying blood to his legs. There was complete blockage of the infrarenal aorta, that delivers oxygenated blood from the heart to the rest of the body and 90% blockage of the left internal carotid.”

Initially, as the patient was not ready for surgery, the medical team attempted a minimally invasive approach with stenting but the blockages were too severe to allow the stent to be placed. The patient was then counselled and he agreed to the surgery. He underwent very rare type of surgery performed in single stage where total arterial bypass surgery of heart and bypass from chest artery to lower limb artery was performed at the same time. This involved a beating heart CABG (Coronary Artery Bypass Graft) with three grafts. Coronary artery bypass grafting (CABG) is a surgical procedure that redirects blood flow around blocked coronary arteries to improve blood supply to the heart muscle.

To address the Peripheral artery disease (PAD) in his legs with the blocked arteries reducing blood flow to the limbs, an extensive bypass of the lower limb arteries was done. The procedure restored blood flow to the legs. These complex procedures required a high level of surgical expertise and careful strategic planning. The patient showed remarkable recovery following the surgery. He was extubated just one day post-operation and mobilized out of bed by the second day. He was able to walk without pain within three days. He expressed his gratitude to the medical team for their expertise and care.
