मुंबई, 18 जानेवारी 2024 (GNI): CAR T cell कायमेरिक अँटीजेन रिसेप्टर T-cell थेरपी इम्युनोथेरपीचा नवीन, आशादायक प्रकार आहे. ज्यामध्ये केमोथेरपी किंवा इतर उपचारांचा काहीही उपयोग होत नाही अशा काही पुढच्या टप्प्यात पोहोचलेल्या ब्लड कँसरमध्ये ही थेरपी उपयोगी ठरते. आता ही थेरपी कोकिलाबेन धीरूभाई अंबानी हॉस्पिटल मुंबईमध्ये उपलब्ध आहे. ही थेरपी प्रत्येक रुग्णानुसार विकसित केली जाते आणि कँसरला टार्गेट करण्यासाठी रुग्णाच्या स्वतःच्या इम्यून सिस्टिम सेल्सना रिप्रोग्रामिंग करून काम करते. या इम्यून सेल्सना टी सेल्स किंवा टी लिम्फोसाईट्स म्हटले जाते, या पेशी संसर्गापासून आपले रक्षण करतात आणि कर्करोगाच्या पेशींना नष्ट करण्यात महत्त्वपूर्ण भूमिका बजावतात. अगदी हल्लीपर्यंत ही थेरपी फक्त युनायटेड स्टेट्स, काही युरोपियन देश, इस्त्रायल व चीनमध्ये उपलब्ध होती. पण नुकतेच केंद्रीय औषधे नियंत्रण मानक संस्थेने (CDSCO) भारतातील पहिल्या स्वदेशात विकसित करण्यात आलेल्या CAR-T cell उत्पादनाला मार्केट ऑथोरायजेशन दिले आहे. स्वदेशात विकसित करण्यात आलेली कायमेरिक अँटीजेन रिसेप्टर (CAR) T-cell थेरपी भारतातील पुन्हा-पुन्हा होणाऱ्या बी-सेल लिम्फोमा किंवा ल्युकेमियाच्या रुग्णांना नवी आशा प्रदान करते. खासकरून ज्यांच्या पारंपरिक थेरपीज करून संपल्या आहेत अशा रुग्णांसाठी हा खूप मोठा दिलासा आहे. कोकिलाबेन धीरूभाई अंबानी हॉस्पिटल मुंबईचे सिनियर कन्सल्टन्ट – हेमेटो ऑन्कोलॉजी आणि बोन मॅरो ट्रान्सप्लान्टेशन डॉ समीर ए तुळपुळे यांनी सांगितले, “ CAR T-cell थेरपी वैयक्तिक दृष्टिकोनातून कॅन्सर उपचारांचा एक प्रकार आहे. भारतात अनेक रुग्णांना या ब्लड कॅन्सरच्या या अत्याधुनिक उपचाराचे लाभ मिळतील, याचा भारतातील खर्च परदेशात येणाऱ्या खर्चाच्या फक्त एक-दशांश इतका आहे. हा अत्याधुनिक उपचार ब्लड कॅन्सरच्या रुग्णांसाठी विशेष तयार करण्यात आलेल्या कॅन्सर देखभालीमध्ये महत्त्वपूर्ण प्रगतीचे एक प्रतीक आहे.” त्यांनी पुढे सांगितले, “यामध्ये अनेक टप्पे असतात. ही थेरपी एफेरेसिस प्रक्रियेमार्फत केली जाते, यामध्ये रुग्णाचे रक्त एका मशिनमार्फत वाहते, जे T cells ना वेगळे करते. या T cells ना कॅन्सरला मारणाऱ्या सुपरचार्ज्ड सेल्स बनवण्यासाठी CAR जोडून प्रयोगशाळेमध्ये जेनेटिक मॉडिफिकेशन केले जाते. त्यानंतर त्यांची संख्या वाढू दिली जाते. पुरेशा पेशी तयार होईपर्यंत काही दिवसांसाठी रुग्णाला सुरुवातीचे उपचार दिले जातात. त्यानंतर पेशी पुन्हा रुग्णाच्या शरीरात सोडल्या जातात. हे उपचार केल्यावर काही प्रतिकूल परिणाम होऊ नयेत यासाठी रुग्णाची १० ते १२ दिवस देखभाल केली जाते. हॉस्पिटलमधून घरी पाठवण्यात आल्यानंतर रुग्णाचे बारकाईने फॉलो-अप केले जाते आणि केमोथेरपी करतेवेळी जी काळजी घेतली होती, त्याचप्रमाणे स्टॅंडर्ड काळजी घेणे आवश्यक असते.”ends GNI
